Completion Card Data

Filling out this simple form makes it possible to present you with your MSF Completion Card upon successfully finishing the Basic Rider Course (if filled out at least a week before class).
(Failure to submit data results in snail mail delivery of Card 4-6 weeks after graduation)

Name -- This is how your name will appear on your MSF Completion Card. Please put it in the following order: Last, First Middle Initial if desired (example: Jones, Michael T.)
