MSF Replacement Completion Card

RidingSafety.com must verify you were trained through us before we can issue a
replacement card. The following information is needed to do that.

DO NOT FILL OUT THIS FORM UNLESS RIDINGSAFETY.COM
TRAINED YOU (ON THE LSU-SHREVEPORT CAMPUS).
ONLY RIDINGSAFETY.COM TRAINED RIDERS WILL
RECEIVE A RESPONSE TO THIS FORM.

RIDINGSAFETY CANNOT / WILL NOT REPLACE
A MSF COMPLETION CARD FOR SOMEONE WE DID NOT TRAIN.**

Nor will RidingSafety replace a card over four years old -- it is time to
get retained to see what has changed and to fix bad habits.

Once RidingSafety.com verifies we trained you, you will receive an email
from us with a link to a web page that allows you to provide your
mailing address and pay the $5 administrative fee. Remember
the replacement card will have the ORIGINAL date
of training on it, not the current date.

**If you were trained at another location under other RiderCoaches you need to contact THAT
TRAINING SITE to obtain a replacement completion card. The RiderCoaches that
trained you will assist you in obtaining a replacement completion card.
